The first time was accidental and may not have actually happened - it was apparently part of some mass hallucination we had...oh, about four months ago? But there's evidence enough to say that we actually went through it, so who am I to say; it mostly involved passing out and waking up elsewhere a few seconds later, though the person I was with at the time said that I most definitely died.
The second time was purposeful, due to poison - I wanted to answer several of the questions you've just asked me, after all. What happens when we die here and everything.
I don't know how I was revived, as no one really would answer that; killjoys, really, the lot of them. But I lost about...oh, twenty-four hours, maybe? It was a day later, anyway, so apparently whatever it is takes time for it to work.
Apparently death negates any major injuries you've sustained, however, assuming that the death occurs shortly after you get the injury - I know someone who's lost an arm and died shortly afterwards. When he woke up, he was perfectly fine. I'm rather certain that one was a death; I saw to it myself.
